How to get duly signed form 19 from previous employer

The company I was working with was under financial crisis and was sacking employees. I was also told to resign. They assured me that they will consider next one month as my notice period without my presence and promised me to pay my full and final settlement amount by one month. Despite several reminders, I have still not received my dues and have not even authorized my form 19. Please suggest me the right course of action.
